• Upper blepharoplasty. Excess skin and puffiness removed via an incision hidden in the natural eyelid crease; outpatient, local anesthetic with mild sedation, about 20 minutes per eyelid; performed for medical necessity (affecting peripheral vision) or cosmetic reasons (eyelid-surgery).
• Lower blepharoplasty. Either transconjunctival (no external incision) or an external incision hidden by the lower lashes; the site states lower blepharoplasty "is not covered by medical insurance as it is a purely cosmetic procedure"; often combined with upper blepharoplasty in the same setting (same page).
• Ptosis repair. Surgical strengthening of the eyelid-lifting muscles; one approach is a minimally invasive "internal ptosis procedure or mullerectomy" performed under the lid with no skin incision (same page).
• Entropion repair. Minimal lower-lid incisions to strengthen muscles and tissue and correct the inward-turning lid, under mild anesthesia, outpatient (same page).

Other procedures

Blepharospasm treatment with Botox injections (efficacy lasts 3-4 months, often covered by insurance), eyelid cancer evaluation and surgery (basal cell, squamous cell, sebaceous cell carcinoma and melanoma, often coordinated with a Mohs dermatologist), chalazion/stye management, tearing evaluation and dacryocystorhinostomy (DCR) for blocked tear ducts, and cosmetic Botox/fillers (eyelid-surgery).

Insurance and cost

Upper blepharoplasty can be billed as medically necessary when it affects peripheral vision, subject to consultation; lower blepharoplasty is stated as not covered by medical insurance, being purely cosmetic (eyelid-surgery).
