Eyelid surgery work

• Upper blepharoplasty. Incision placed in the natural eyelid crease to remove excess skin and fat and adjust supportive muscles; can be combined with endoscopic brow lift (upper-blepharoplasty).
• Lower blepharoplasty. Fat removed or repositioned through a lash-line or transconjunctival incision; in older patients the lateral canthal tendon is tightened; surgery takes one to three hours under local anesthesia or IV sedation (lower-blepharoplasty).
• Ptosis repair. Levator muscle shortened, with redundant skin removed if needed; performed in-office or outpatient under light sedation and local anesthesia, generally under an hour (eyelid-ptosis).
• Brow lift. Endoscopic approach (short incisions behind the hairline, about an hour under light sedation) and direct approach (single incision above the brow, 30-60 minutes, for patients with significant skin laxity) (brow-lift).

Insurance and cost

Upper blepharoplasty: may be medically necessary if excess skin reduces peripheral vision; the practice documents the exam for insurance to decide. Ptosis repair: often medically necessary; the practice submits evaluation, photographs and documentation for insurance review. Brow lift: may be covered if brow position is extremely low and causes peripheral vision loss. No prices are published on any of these pages.

Recovery: upper blepharoplasty patients avoid rigorous activity, bending and heavy lifting for 1-2 weeks; incision scar fades about six months; final healing about a year, with improvement up to two years. Lower blepharoplasty: most bruising and swelling fade in 2-4 weeks, return to work in 1-2 weeks, normal exercise and makeup resume in 2-3 weeks, swimming avoided one month, final results at 6 months. Ptosis repair: light activity resumes within a few days, strenuous activity avoided a couple of weeks, initial results in 2-4 weeks, final results over a few months. Brow lift: endoscopic approach about one to two weeks of healing, one week off work; direct approach 10-14 days, brow position settles over about six months.
